Have a place for billing and a mobile app.

I think the idea is good but you should allow it to sync to a phone so you can have your records with you when you travel. Things like immunizations and allergies would be very helpful on the road. Also have a place for the billing information such as what was billed to you and the insurance company, check numbers, balance, etc.
